NDIS Support Coordination is an NDIS funding service that can help you with the day-to-day coordination of your NDIS supports as well as connecting you to community and mainstream services. Caring Hands Community Support Services is to provide all three levels of Support Coordination, which include Support Connection, Support Coordination and Specialist Support Coordination.
If your plan includes funding for Support Coordination, Direct Care Australia can assist you in making your NDIS Plan a reality. With so many NDIS options and service providers available, deciding which services are right for you – and how to connect to them – can be difficult. This is where we can assist you. We assist you in carrying out your NDIS plan by utilising your preferred services and providers. As Support Coordinators, we are obligated to give you unbiased advice and provide guidance to ensure that you get the most out of your NDIS
There are three levels of support coordination that can be included in your plan:
Support Connection
Support Connection is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals. This is essential for the participant to feel comfortable and confident in using their plan. We will work with you to identify your community networks and support you to develop these relationships.
Support coordination – coordination of supports
Support Coordination is the coordination of supports that will help you develop the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure that a variety of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Specialist support coordination
Specialist Support Coordination is a higher level of support coordination aimed specifically at people in more complex situations who require specialist assistance. A Specialist Support Coordinator will help you manage challenges in your support environment while also ensuring consistent service delivery and positive outcomes. If you are eligible for Specialist Support Coordination, your support coordinator will work with you and your significant others to develop a comprehensive support plan. This support plan will detail all of the services and supports you need to achieve your goals.
Caring Hands Community Support Coordination team can help with:
Managing NDIS budgets to make the most of your plan.
- Liaison with the NDIA to obtain the necessary support and funding. This includes lodging plan reviews when the amount of funding in your current plan is incorrect or insufficient.
- Review and Monitoring.
- Obtain new specialised and customised equipment or repair old equipment through the use of NDIS funding.
- Attend meetings with you to provide support and/or advocacy where you request. Gaining your consent before implementing any service by listening to your needs and preferences.
